About the Role

We are seeking a dedicated RN Case Manager to conduct non-clinical in-person visits with members in their homes, assisted living facilities, and nursing homes, following Federal guidelines. You will play a key role in completing UAS Assessments to support care management, with a minimum caseload of 15 assessments per week.

Key Responsibilities

Conduct face-to-face non-clinical assessments to evaluate members' functional status, medical, behavioral, psychosocial, and community resource needs.

Provide detailed assessment findings to the Interdisciplinary Care Team (ICT) and act as a liaison to ensure members receive appropriate care.

Develop and adjust Individualized Care Plans (ICP) and Person-Centered Service Plans (PCSP) based on members' clinical, behavioral, and social needs.

Facilitate care transitions and coordinate necessary services for member care and discharge planning.

Educate members, providers, and community agencies about medical benefits, policies, and procedures.

Participate in clinical case reviews and contribute to continuous improvement efforts.

Maintain knowledge of community resources, payer requirements, and network services to support member needs.

Collaborate with other Care Managers to standardize clinical practices, enhance care delivery, and develop effective policies and procedures.

Provide support to fellow Assessment Nurses when needed.

Utilize data insights to improve care coordination and delivery.

Qualifications & Requirements

Active RN License in the state of New York

UAS Certification (preferred); however, candidates with home care management experience will be considered even if they are not UAS certified.

Bachelor's Degree in Nursing, Health Education, or a related field

New graduates are welcome to apply

Bilingual candidates preferred, but not required

Training & Orientation

A 5-day orientation will be provided for RNs who are already UAS certified.
