RN Home Health for our Integrated Health Services (Skilled home-based-care) department. This position is
Full-time: Monday - Friday 8:00am - 5:00pm with flexible scheduling up to 40 hours per week. CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required. The
RN Home Health will Manage the overall Plan of Care for a case load of Integrated health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient/caregiver problems, establish nursing goals, and interventions. Our
RN Home Health will work with physician to establish medical Plan of Care for the patient. They will establish and maintain therapeutic relationship with patient, including providing disease education, medication management, and health recovery.
The
RN Home Health will work closely with other members of the interdisciplinary team and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Our
RN Home Health will document the goals and outcomes of care. They assume on call responsibilities as scheduled. Requirements Education and Experience : - Graduate of an accredited School of Nursing
- Must be currently licensed as an RN in North Carolina and in good standings with NCBON
- One year of experience in an acute, hospice, home health setting, or long-term care setting. Two years of experience preferred.
- Bachelor's degree in nursing preferred.
- Patient management skills needed include excellent patient assessment and clinical nursing skills, critical thinking, problem solving, delegation, effective interpersonal and communication skills necessary.
- CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
- Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
