Adelphi is seeking a Registered Nurse to provide part-time services for a healthcare initiative in Stamford, Connecticut. This role is focused on chronic disease prevention, public health education, infectious disease management, and community outreach. Requirements:
• Active Connecticut RN License (must be current and in good standing).
• CPR Certification is required.
• Valid Driver’s License.
• Experience in Public Health, Community Health, or Preventive Care Settings preferred.
Responsibilities:
• Conduct chronic disease screenings, management, and health education (e.g., diabetes, hypertension, obesity).
• Provide immunizations and infectious disease prevention education.
• Perform STD screenings, assessments, and patient education following CDC guidelines.
• Conduct Tuberculosis (TB) screenings, referrals, and follow-ups as per state and CDC health protocols.
• Carry out early periodic screening, diagnosis, and treatment (EPSDT) for infants, school-aged children, and adults.
• Administer vaccinations, health screenings, and referrals for patients needing further medical interventions.
• Engage in community-based public health outreach programs.
• Collaborate with healthcare professionals and social workers to implement public health programs.
• Educate patients and families on health conditions, medications, and disease prevention.
• Maintain electronic medical records (EMR) and ensure proper data documentation.
• Participate in public health training, compliance measures, and ongoing city, state, and federal health initiatives.
