We're looking for a dedicated LPN Care Coordinator to support patients with chronic conditions, ensuring they receive the right care at the right time—all from the comfort of your home.

What You'll Do:
Review patient EMR records to determine eligibility for care management services.
Conduct proactive outreach (monthly calls) to provide education, support, and resource connections.
Develop and maintain individualized care plans to ensure patients receive comprehensive care.
Monitor patient progress and collaborate with care teams to close care gaps.
Ensure compliance with Medicare, HIPAA, and care management protocols.
Engage in high-volume patient outreach - this is a phone-based role requiring daily interaction. You'll be expected to meet performance targets, ensuring each patient receives at least 20 minutes of care coordination per month.

What We're Looking For:
✅ Active LPN Compact License (or ability to obtain one if needed).
✅ 5+ years of patient-centered care experience with strong clinical knowledge.
✅ Tech-savvy with EMR proficiency and comfortable in a digital healthcare environment.
✅ Excellent communication skills - you'll be engaging with patients daily over the phone.
✅ Self-motivated and detail-oriented - you take ownership of your work and consistently meet performance goals.
✅ Fluency in Spanish is a plus - bilingual candidates are highly encouraged to apply!
