Schedule: Monday - Friday, 1:00 PM - 9:00 PM 40 hours per week. Must meet minimum productivity of 30 points per week

Shift will include on call and scheduled visits as needed.

Key Responsibilities

  • Perform comprehensive assessments of patients in their home environments.
  • Design, implement, and modify individualized care plans in collaboration with the healthcare team.
  • Administer prescribed medications and treatments appropriately and safely.
  • Monitor and document patients' progress, making adjustments as necessary.
  • Provide education to patients and families on health conditions and care plans.
  • Facilitate communication and coordination among team members and other healthcare providers.
  • Offer emotional support and guidance to patients and families throughout the care process.

Requirements

  • Current RN license in the state of practice.
  • Experience with HomeCare Homebase Charting strongly preferred.
  • Strong clinical assessment skills and critical thinking capabilities.
  • Excellent communication skills and the ability to build rapport with patients.
  • Self-motivated with strong time management skills.
  • BLS certification required.
  • OASIS experience required.
  • Must have at least 12 months of home health experience within the past 3 years.
