Job Description

Are you a compassionate and driven healthcare professional passionate about empowering individuals to achieve their optimal health? We're seeking a dedicated Health Coach Care Coordinator to join our team and make a real difference in patients' lives. In this dynamic role, you'll provide personalized support and guidance to patients managing new or existing chronic conditions, all from the comfort of your virtual workspace.

Your Mission:

As a Health Coach Care Coordinator, you'll be a trusted partner in your patients' wellness journey. You'll provide comprehensive education on disease processes, self-management strategies, and the importance of lifestyle changes, including diet and exercise. Your ability to build rapport and effectively address roadblocks will be crucial in helping patients reach their health goals.

What You'll Do:

Empower Through Education: Deliver thorough and personalized education on disease processes, self-management strategies, and healthy lifestyle choices, including physical activity and nutrition.
Provide Virtual Support: Engage with patients regularly via our user-friendly virtual platform, offering support through digital communication, phone calls, video consultations, and text messaging.
Facilitate Meaningful Conversations: Comfortably guide patients through difficult conversations, addressing their concerns and providing clear, accurate information.
Personalized Care Coordination: Recognize individual patient needs and tailor interventions accordingly, utilizing Motivational Interviewing (MI) techniques to address challenges.
Resource Navigation: Direct patients to relevant resources and support systems to enhance their wellness journey.
Meticulous Documentation: Maintain accurate and detailed patient records, including interventions, provider interactions, and clinical notes.
Collaborate and Grow: Work effectively as part of a team, proactively seeking information and embracing opportunities for professional development.
Adaptable and Responsive: Be responsive to patient communications and adaptable to evolving needs and responsibilities.
Schedule:

Full-time, 8-hour shifts, Monday through Friday.
Qualifications:

Required:
High school diploma or equivalent.
Minimum of 2 years of experience as a Medical Assistant or other Healthcare Professional in a healthcare setting, providing direct patient care.
Strong knowledge of technology and comfort using virtual communication platforms.
Must be willing to complete a background check.
Must live a fit and healthy lifestyle.
Health coaching certification strongly preferred, or desire to obtain one upon employment. (Company Paid)
Preferred:
Associate's degree or higher in Medical Assisting or a related healthcare field.
Current certification from a nationally recognized healthcare organization, or willingness to renew a prior certification.
Experience in health coaching or consultation.
Employment Type: Full-Time
Salary: $ 43,561.00 Per Year
