YSS is seeking a compassionate  Outpatient Clinic Nurse  to join our Clinic Operations team in Ames. In this role, you will be part of a multidisciplinary team to assess, plan, develop, coordinate and provide treatment, rehabilitation, and support services to a telehealth team in an outpatient program. RN/LPN supports the prescribing professionals/patient/agency with medication management and referrals as necessary for additional services.

The RN/LPN functions as a clinical member of the team to provide nursing assessment and care, oversight of medications, support, and other rehabilitative services to the patients. Other responsibilities include:

  • Use available data, such as assessments, participant evaluations, and service observations to identify health and wellness needs to develop individualized care coordination plans.
  • Evaluate and chart psychotropic medication effectiveness, complications, and side effects.
  • Organize and manage the system of getting medication to patients and integrate medication into patient’s individual treatment plan. 
  • Support ongoing collaboration between physical health and behavioral health providers.
  • Must managed and communicate with Pharmacy’s and handle phone calls related to medication refills. 
  • Must coordinate and perform injections as needed for patients as necessary.
  • Responsible for managing and monitoring the refrigerated medications and complying to regulations on refrigerated medication. 
  • Responsible for preparing and administering vaccinations, including flu shots.
  • Provide health education to clients on preventing and managing chronic conditions.
  • Monitor high-risk patient cases and facilitate communication with prescribing professional as necessary.
  • Professionally and effectively present information in person or by the phone and respond to questions from patients, medical professionals, and management staff.
  • Develop symptom response and crisis plans as necessary to ensure more appropriate use of physician visits rather than ER use and hospital admissions.
  • Schedule and coordinate initial, crisis, intake and other appointments with clients.
  • Complete required documentation for each patient according to agency policy.
  • Attend regularly scheduled meetings and training sessions to enhance performance.
  • Multitasking and ability to quickly manipulate technology as needed for telehealth services.
  • Requires the ability to use multiple computer programs, multi-task and handle phone calls.
  • Accomplish all tasks as appropriately assigned or requested.

MUST HAVES - Required Qualifications

  • Currently licensed as a RN / LPN in the State of Iowa.
  • Experience working with youth facing substance use and dependency.

SHOULD HAVES - Preferred Qualifications

  • Customer/client focus and understands the basic principles of providing quality service.
  • Is a strong communicator both verbally and non-verbally, with excellent observational, listening, speaking, and writing skills.
  • Able to effectively communicate with a variety of audiences including customers, clients, and the public as well as colleagues and other staff members
  • Patient and calm under pressure and able to deal with challenging situations with a good attitude. 
  • Can think and act quickly to resolve complex problems using creative/critical thinking, resourcefulness, and tact 
  • Organized with a high attention to detail.
  • Able to manage time and resources effectively.
